The subscriber understands that all records regarding him or herself are considered the property of FAR, LLC.

The extent of information recorded is the subscriber’s name, physical address, email address, phone number, contact information, payment process, and pertinent communication given to the therapist/spiritual coach.

Confidentiality is strict, however, Florida Statute, Chapter 394 governs reporting and access to certain data of confidential information that may be required through court purposes and State statutes.

In order to obtain a released copy of your records by FAR, LLC or assigned to an agency or professional, the subscriber must request in writing, with his/her signature appropriately witnessed and notarized.

The cost of a copy of the returned records will be incurred by the subscriber.
